How do I get a non-medical exemption for my child?

Step One: Call Oakland Connects Helpline at 1-800-848-5533 to schedule an appointment.

The child's parent or legal guardian can schedule an appointment by calling the Oakland Connects Helpline (formally Nurse on Call) at 1-800-848-5533. Appointments are scheduled on a first-come, first-served basis for Oakland County residents and/or for students attending Oakland County schools or early learning centers. Exemption appointments are offered throughout the entire year at both Oakland County Health Division Pontiac and Southfield locations. The exemption session is for the parent/legal guardian. Children are welcome, but they are not required to attend.

Step Two: Attend the exemption education session at no cost, in a private setting, with a Public Health Nurse at Oakland County Health Division.

The session typically lasts 30 minutes but can vary depending on the number of children in your family. Once the exemption education session and all non-medical exemption form requirements are complete, the non-medical exemption will be issued.

What You Need for Your Appointment

  • Picture ID
  • Guardianship papers, if applicable
  • Official immunization record for vaccines that have not been recorded in the Michigan Care Improvement Registry (MCIR)
  • Name of school/early learning center that your child will attend

Immunization Requirements for School and Early Learning Information

Since 1978, Michigan law has required children enrolled in early learning centers or schools to be up to date with required immunizations on or before the first day of school or early learning. Early learning centers are required to report all children enrolled in their centers and schools are required to report all Kindergarten, 7th grade, and newly enrolled students to the district.

The following circumstances allow a required vaccine to be exempted or delayed:

  1. Medical Exemption: A valid medical exemption to a vaccine exists, preventing the child from receiving the vaccine at this time. A Medical Exemption Form is required to be completed and signed by the child's physician (must be an MD or DO) and should state the exemption, vaccine involved, and time period during which the child is preventing from receiving the vaccine.
  2. Non-Medical Exemption: The parent or legal guardian chooses to waive one or more vaccines due to a religious or philosophical belief. In December 2014, Michigan passed an administrative rule which require non-medical exemptions to be obtained from the local health department after an education session with the parent/legal guardian. The 30-minute education session will include information about:
  • Vaccine preventable diseases
  • Required vaccines for school or early learning
  • Exclusion policy during a vaccine preventable disease outbreak
