Early on is an early intervention program for children birth to age three with a developmental delay or an established health condition.

This program provides the following:

• Case management, education and referrals
• Infant-Toddler growth and development assessments
• Individualized Family Service Plan

Public Health Nurses provide home visits to help women:

• Have a healthy pregnancy and a healthy baby
• Become a better parent
• Build a strong network of support for you and your baby
• Make your home a safe place for your baby to live and play
• Get referrals for healthcare, childcare, job training and other support services in your community

Referrals are being accepted now.

Who Can Enroll

• Women who live in the Oakland County, and
• Are enrolled in WIC, or
• Are eligible for Medicaid or the Healthy Michigan Plan, or
• Are uninsured

We encourage women to call us while pregnant, but you can call us after delivery as well.
