• Verification of Birth is needed to initially add child to benefits in Workday.
• Birth certificate and social security number are needed within 45 days of birth to finalize the addition of coverage. This information can be emailed to benefits@oakgov.com

• Finalized Order of Adoption
• Social Security Number

Documents/information required to remove dependents after a divorce:

• Finalized Divorce Decree

Documents/information required to add coverage for yourself and/or dependents after a loss of primary healthcare coverage:

• Documentation from current carrier confirming that coverage is terminating for the individual(s) and the date that coverage will be terminated.
• Official Marriage Certificate (to add spouse and/or stepchildren)
• Birth Certificate (to add children and/or stepchildren)
• Social Security Number
