FMLA, Disability and Life/AD&D insurance plans are administered through Unum.

Starting October 1, 2025, Unum will administer FMLA and Disability claim requests through their Total Leave Portal.  Please see below for more information.

FMLA and Disability

Whether you are requesting leave through the Family and Medical Leave Act (FMLA) and/or short-term disability, your first step is to submit a claim through Unum's Total Leave Portal.  You are also required to keep your department informed of leave dates and anticipated return-to-work dates.  You are not required to disclose any personal health information to anyone at Oakland County when requesting leave.
