DNA Testing Requests

Requests for DNA testing must be made in writing and include the following:

  1. Name of person requesting specimen, phone #, email, address, and relationship to decedent.
  2. State specifically what you are requesting (Paternity/DNA testing, genetic banking, type of specimen*, etc.)
  3. The decedent’s name, date of death, and case number.

*DNA/Paternity tests usually require a blood spot card. Genetic testing usually requires a whole blood specimen.

Upon receipt of your written request, we will respond with confirmation of having retained a specimen usable for testing or inform you that there is no specimen available.

If there is specimen available for additional testing, we will require the following:

  1. Completed “Authorization for Release” form from legal next-of-kin or Court Order to provide specimen for testing.
  2. Please contact the lab of your choosing as soon as availability of the specimen is determined to facilitate transfer. Most labs require shipping kits or have additional protocols that are required for timely testing.  Have the lab contact the OCMEO directly to facilitate the transfer.
