The judge has ordered you to submit to a presentence investigation (PSI), conducted by the Probation Department prior to sentencing. Immediately after you have pled or been found guilty, you will have appeared at the Probation Department and received a scheduled appointment for the PSI interview. For your convenience and to reduce your time at the Court, all required paperwork has been placed on the website for you to fill out online.

You are required to fill out, IN FULL, the Client Basic Information (CBI) form at this link: CBI FORM. You will have to save the completed document and email it to 524probation@oakgov.com. You can also print it and deliver it to the probation department. Be sure to include your statement, which reflects what happened the day of the offense.  You must also fill out the NEEDS assessment found at this link NEEDS assessment. Once you submit it, it will be directly sent  to the Court.

These forms can only be filled out on a computer or laptop. They cannot be filled out on a cellular phone or tablet. All paperwork is due no less than five business days prior to your appointment. If you did not receive an appointment day or time before leaving the  Court, contact the probation department immediately.

Failure to turn in your completed paperwork and appear for your appointment on time will affect your ability to be interviewed, which may also delay your sentencing date.
