Please follow ALL 4 STEPS in order to complete your request for PPO.

  • Step 1. Complete PPO Petition and Proposed Order by using Michigan Legal Help.
    • OR Fill out the petition/form that is appropriate for your situation that can be found below this subject page.
    • Note:  Michigan Legal Help may not be compatible with MAC computers.
  • Step 2. CompleteVerified Statement.
  • Step 3. CompleteParenting Time Worksheet if you have children in common with Respondent.
  • Step 4.  Send ALL DOCUMENTS in Step 1-3 to PPO@oakgov.com

* YOU MUST include your contact information on your PPO petition to ensure that your rights are protected. Without this information, the court will not be able to schedule a hearing for you in the event your PPO is denied.

Below is a list of forms (in order by form number) developed by the State Court Administrative Office (SCAO) for use in preparing Personal Protection Orders (PPO). These are pdf files that have been customized for use in Oakland County. They may be completed in Adobe or printed and completed.

Note: The most recent revision of SCAO approved forms for use anywhere in Michigan is also available.

Petition for Personal Protection Order (Domestic Relationship) - CC 375:
This form is used when you and the person you want restrained have a "domestic relationship" (i.e., husband and wife, dating relationship, household resident).

Petition for Personal Protection Order Against A Minor (Domestic Relationship) - CC 375M:
This form is used when you and the minor you want restrained has a "domestic relationship" (i.e., a minor in a dating relationship).

Personal Protection Order (Domestic Relationship) - CC 376:
This companion form to CC 375 becomes the order that is signed by the judge.

Personal Protection Order Against A Minor (Domestic Relationship) - CC 376M:
This companion form to CC 375M becomes the order that is signed by the judge.

Petition for Personal Protection Order Against Stalking (Non Domestic) - CC 377:
This form is used if you do not have a "domestic relationship" with the person you want restrained and they have been harassing or stalking you.

Petition for Personal Protection Order Against Stalking By A Minor (Non Domestic) - CC 377M:
This form used if you do not have a "domestic relationship" with the minor you want restrained and they have been harassing or stalking you.

Motion and Order to Dismiss Action for Personal Protection Order - CC 378

Motion to Modify, Extend or Terminate Personal Protection Order - CC 379:
This form modifies, dismisses, or extends the duration of a PPO. Also, the Respondent may use this form to try to change the terms of the order or to terminate the restraint.

Personal Protection Order Against Stalking (Non-Domestic) - CC 380:
This companion form to CC 377 becomes the order that is signed by the judge.

Personal Protection Order Against Stalking By A Minor (Non Domestic) - CC 380M:
This companion form to CC 377M becomes the order that is signed by the judge.

Notice of Hearing On Petition for Personal Protection Order - CC 381:
This document is used to inform all interested parties of a Personal Protection Hearing that has been scheduled.

Motion and Order To Show Cause for Violating Valid Personal/Foreign Protection Order - CC 382:
This document is filed if the Respondent has been served with the PPO and has violated the terms of the order. This motion orders the Respondent into court where he/she faces a contempt charge.
