If you are the victim or victim relative of a crime that is being prosecuted through the Oakland County Prosecutor's Office, and you have a Prosecutor Order Number (also known as a P.O. number) you can fill out the Crime Victim Services Rights Request form. This form gives you a list of rights you may request our section to provide. By answering the questions accordingly we can keep you informed of upcoming court hearings and the final disposition of the defendant.
Without a Prosecutors Order Number we CANNOT process your form. If you have lost or forgotten yours, please contact us by telephone and we will attempt to assist you.
When filling out the form please circle the appropriate response to each question as well as giving us your name, full address including state and zip code and a phone number that we can reach you at. You must have a Prosecutor Order number and the defendant's name. The Prosecutor Order number is different from the Incident Number that is on the police report.
