Guardianship of Adult
A Guardian is a person who is given Probate Court authority to be responsible for the personal and physical well being of an adult who is called a Legally Incapacitated Individual (LII). The Guardian has the same powers and duties over that LII as parents have over their children. A prospective Guardian may be nominated by petition (filed with the Probate Court) or may be named in a will.

Below are the forms that must be completed to start an adult guardianship proceeding in Oakland County Probate Court. These forms are insertable pdf documents that may be completed in Adobe or may be printed, then completed.

Forms to start a guardianship may be mailed to the above address provided they contain all signatures and include the appropriate filing fee(s). The filing fee to start each guardianship is $150.00 cash or check (please do not mail cash).

Forms During Pendency of an Adult Guardianship
The following document is used after a guardian is appointed and must be filed each year. This document can be mailed to Probate Court at the address given above. There is no fee to file this document.

Forms to Modify or Close an Adult Guardianship
The Filing Fee to petition to modify or terminate each adult guardianship case is $20.00. The following document is used to ask the court's permission to modify or close a case. It can be mailed to the address given above as long as the filing fee is included. It will be set for hearing.
