Below are the forms that must be completed to start a formal or supervised estate proceeding in Oakland County Probate Court. These forms are insertable pdf documents that may be completed in Adobe or may be printed, then completed.

Some of these forms (primarily those with a form number beginning PEMH) reside on the Oakland County Probate Court's web site and the file size will be listed. Other forms (most documents having form numbers beginning with "PC") are linked directly to the form on the State Court Administrative Office web site.

Forms to start a formal or supervised estate may be mailed to the above address provided they contain all signatures and include the appropriate filing fee(s). There must be a separate file and fee for each individual.
The filing fee to start a formal or supervised estate is $150.00.
A Gross Estate Fee based upon the Inventory Value of the assets before funeral expenses (Examples of the Gross Estate Fee: $25.00 for $1,000 in assets; $68.75 for $10,000 in assets; $118.75 for $20,000 in assets) will be needed before you can close the estate.

If you choose to have your matter allowed without a hearing, you will need to obtain full Waiver/Consent (PC 561) from all interested persons. You will be required to complete and present a Bond Information Form (PEMH 1001; file size 11k) for the Juge to determine if a bond is necessary.
