If you answer YES to any of the following questions, you may be at risk for delivering a preterm infant, this can cause infant death.

• Do you miss doctor visits while pregnant?
• Do you skip taking your prenatal vitamins?
• Do you use alcohol, tobacco or drugs?
• Are you younger than 18 or older than 35?
• Do you or have you had a sexually transmitted disease?
• Have you had a miscarriage?
