Approved SCAO (State Court Administrative Office) forms are available through the Michigan Courts website. You may also find forms at the Oakland County Law Library.
The Oakland County Clerk's Office does not provide legal forms.

The Oakland County Bar Association can be contacted at 248-338-2100 or by email at irs@ocba.org.

Where do I get a Personal Protection Order (PPO)?
PPO's (Personal Protection Orders) are filed at the Oakland County Clerk's Office in the Legal Records division on the ground floor of the courthouse. You must appear in person to obtain a PPO. The Women's Survival Center has staffed the PPO Office to assist citizens in filling out forms and answering questions.
Forms are available at the PPO Office located in Juvenile Intake on the ground floor in the east wing. You may also obtain forms and instructions online.
