Vital Records/DD214 - Request by Mail

To obtain a certified copy of your DD214 by mail that has been previously recorded with the Oakland County Clerk's office:

1. You must print, complete, and mail the request form (file size 14k).  Please do not forget to sign the form.

2. You must also mail a copy of your valid driver's license or state ID.

3. The first certified copy is FREE and $1.00 for each additional copy. A money order is the only acceptable payment by mail.

4. Place your completed request form, copy of valid drivers license or state ID, and money order (if necessary) in an envelope addressed to:

             Oakland County Clerk's Office
             1200 N. Telegraph Rd. Dept #413
             Pontiac, MI  48341
             Attn: Vital Records

Your DD214 will be returned to you by regular mail.
