Name Change Petitions are available through the Oakland County Clerk's Office.

You must be a resident of Oakland County for at least one year in order to petition the court for a name change. Children must be over the age of one year to process a last name change.

Complete instructions for name change procedures are as follows:

• 21 years of age or younger (file size 24k)
• 22 years of age or older (file size 26k)

Submit your completed Name Change Petition (file size 124k) to the Oakland County Clerk's Office in person or by mail. Hours are Monday-Friday 8:00 a.m. to 4:30 p.m.  Contact us
