State Court Administrative Office--Forms for Personal Protection Orders

Below is a list of forms (in order by form number) developed by the State Court Administrative Office for use in preparing Personal Protection Orders.  These are .pdf files that have been customized for use in Oakland County.  They may be completed in Adobe (type-in) or may be printed, then completed.

CC 375 - Petition for Personal Protection Order (Domestic Relationship) (file size 461k) This form is used when you and the person you want restrained have a "domestic relationship" (i.e., husband and wife, dating relationship, household resident).

CC 375M - Petition for Personal Protection Order Against A Minor (Domestic Relationship) (file size 127k)  This form is used when you and the minor you want restrained has a "domestic relationship" (i.e., a minor in a dating relationship).

CC 376 - Personal Protection Order (Domestic Relationship) (file size 16k)  This companion form to CC 375 becomes the order that is signed by the judge.

CC 376M - Personal Protection Order Against A Minor (Domestic Relationship) (file size 18k)  This companion form to CC 375M becomes the order that is signed by the judge.

CC 377 - Petition for Personal Protection Order Against Stalking (Non Domestic) (file size 125k) This form is used if you do not have a "domestic relationship" with the person you want restrained and they have been harassing or stalking you.

CC 377M - Petition for Personal Protection Order Against Stalking By A Minor (Non Domestic) (file size 119k) This form used used if you do not have a "domestic relationship" with the minor you want restrained and they have been harassing or stalking you.

CC 379 - Motion to Modify, Extend or Terminate Personal Protection Order; (file size 108k)  This form modifies, dismisses, or extends the duration of a PPO.  Also, the Respondent may use this form to try to change the terms of the order or to terminate the restraint.

CC 380 - Personal Protection Order Against Stalking (Non-Domestic) (file size 14k) This companion form to CC 377 becomes the order that is signed by the judge.

CC 380M - Personal Protection Order Against Stalking By A Minor (Non Domestic)  (file size 17k) This companion form to CC 377M becomes the order that is signed by the judge.

CC 381 - Notice of Hearing On Petition for Personal Protection Order; (file size 80k) This document is used to inform all interested parties of a Personal Protection Hearing that has been scheduled.

CC 382 - Motion and Order To Show Cause for Violating Valid Personal/Foreign Protection Order (file size 101k) This document is filed if the Respondent has been served with the PPO and has violated the terms of the order.  This motion orders the Respondent into court where he/she faces a contempt charge.
