Forms for Personal Protection Orders-Oakland County (In-House)

Below is a list of In-House forms required by Oakland County for use in preparing various types of Personal Protection Orders.  These are .pdf files that may be completed in Adobe or may be printed, then completed.

CC-PPO 1 - Proof of Service (file size 115k) This document is filed with the Court after the respondent has been served with a copy of the PPO.

CC-PPO 2 - Verified Statement re: PPO Action (file size 131k) This form verifies that you wish to file for a PPO and provides additional information regarding your relationship with the respondent.

CC-PPO 3 - Prior Cases (file size 100k) This form provides the court with information regarding other court cases involving you and/or the respondent.

CC-PPO 4 - Worksheet re: Custody and Parenting Time (file size 131k) This worksheet helps the court to decide what arrangements should be made if the PPO affects parenting and custody. 

CC-PPO 11 - Addendum to Petition for Personal Protection Order (Domestic) (file size 140k) This form provides additional information about the actions of the respondent to support your request for a PPO where a domestic relationship exists.

CC-PPO 12 - Addendum to Petition for Personal Protection Order Stalking (file size 20k)  This form provides additional information about the actions of the respondent to support your request for a PPO where a domestic relationship does not exist. 

CC-PPO 13 - Addendum to Petition Personal Protection Order (Domestic-Juvenile) (file size 45k)  his form provides additional information about the actions of a minor respondent to support your request for a PPO where a domestic relationship with the minor exists.

CC-PPO 14 - Addendum to Petition Personal Protection Order Stalking-Juvenile (file size 31k) This form provides additional information about the actions of a minor respondent to support your request for a PPO where a domestic relationship with the minor does not exist.
